New document templates
Based on the Regulation No. 35 of the Rector of the University of Lodz of 15 December 2025 the following have been changed:
- the application template to the Committee,
- the informed consent form for research participants.
Where can the downloadable documents be found?
The new application form and attachments can be downloaded:
- from the Research Ethics tab on the university website,
- from the Employee Portal: Employee Knowledge Base.
How to submit an application?
To submit an application, you have to:
- complete the new form,
- collect the required signatures,
- scan the documents,
- send them to: komisja.etyka@uni.lodz.pl.
The applications are accepted only in an electronic form.
